Transition Specialist

Job Summary

A company is looking for a Transition Specialist to support the coordination of member transitions in the community.

Key Responsibilities

• Support care coordination teams to develop effective transition plans for community members
• Assist with member transitions based on enrollment or care service changes
• Track transition metrics and ensure proper documentation of care transitions

Required Qualifications

• Bachelor's degree required
• 2 - 4 years of related experience preferred
• Equivalent experience may be considered in lieu of degree
• Experience in care coordination or health services is beneficial
• Knowledge of community health resources and services is advantageous
