Job Summary

A company is looking for a Clinical Pharmacist Patient Care - Remote.

Key Responsibilities

• Perform medication reviews and develop patient-centered care plans
• Conduct medication reconciliation for high-risk patients and collaborate with care teams
• Implement pharmacy and medical strategies to achieve business goals

Required Qualifications

• Graduation from an accredited College/School of Pharmacy with a B.S. or PharmD
• Active, unrestricted Pharmacist License in applicable state(s)
• 1+ years of Post-Graduate/Residency training or equivalent 3+ years of practice
• Proficiency with Microsoft Office Tools and EMRs
• Willingness to obtain additional state licenses as requested
