Job Summary

A company is looking for a Social Worker to provide psychosocial support and care planning for patients with chronic kidney disease.

Key Responsibilities

• Participates in care planning with patients and healthcare teams to improve quality of life
• Assesses patient knowledge and social determinants of health affecting treatment adherence
• Provides education and counseling on chronic kidney disease and assists with referrals to community resources

Required Qualifications

• Master's degree in social work
• Active and unrestricted state-specific licensure as a Licensed Clinical Social Worker
• Experience in a renal setting working with ESRD and CKD patients
• Basic computer skills and understanding
• Strong understanding of insurance, Medicare, and Medicaid processes
