Bilingual Care Coordinator

Job Summary

A company is looking for a Bilingual Care Coordinator to support care coordination functions and advocate for members' health needs.

Key Responsibilities

• Conduct health risk assessments through member outreach to understand their health and needs
• Coordinate with provider offices and home health agencies to ensure members receive necessary support
• Participate in interdisciplinary rounds and review care plans to provide additional support to members

Required Qualifications

• Minimum education: High school diploma or GED equivalent
• 2+ years of experience in healthcare administration or home care
• Proficient in technology (experience with Gmail/G Suite)
• Previous experience in social services
• Fully bilingual (English/Spanish)
