Job Summary
A company is looking for a Bilingual Care Coordinator to support care coordination functions and advocate for members' health needs.
Key Responsibilities
- Conduct health risk assessments through member outreach to understand their health and needs
- Coordinate with provider offices and home health agencies to ensure members receive necessary support
- Participate in interdisciplinary rounds and review care plans to provide additional support to members
Required Qualifications
- Minimum education: High school diploma or GED equivalent
- 2+ years of experience in healthcare administration or home care
- Proficient in technology (experience with Gmail/G Suite)
- Previous experience in social services
- Fully bilingual (English/Spanish)
