Bilingual Care Coordinator

Job Summary

A company is looking for a Bilingual Care Coordinator to support care coordination functions and advocate for members' healthcare needs.

Key Responsibilities

• Conduct health risk assessments and coordinate with provider offices to ensure member support
• Check in with members to assess their well-being and review care plans for additional support
• Participate in interdisciplinary rounds and provide operational support to the complex care department

Required Qualifications

• Minimum education: High school diploma or GED equivalent
• 2+ years of experience in healthcare administration or home care
• Proficient in technology, with experience in Gmail/G Suite
• Fully bilingual (English/Spanish)
• Comfortable working in an interdisciplinary team environment
