A company is looking for a Bilingual Care Coordinator to support care coordination functions and advocate for members' healthcare needs.
Key Responsibilities
- Conduct health risk assessments and coordinate with provider offices to ensure member support
- Check in with members to assess their well-being and review care plans for additional support
- Participate in interdisciplinary rounds and provide operational support to the complex care department
Required Qualifications
- Minimum education: High school diploma or GED equivalent
- 2+ years of experience in healthcare administration or home care
- Proficient in technology, with experience in Gmail/G Suite
- Fully bilingual (English/Spanish)
- Comfortable working in an interdisciplinary team environment
