Key Responsibilities

- Manage a caseload and utilize technology for effective coaching and resource referral
- Assess caregiver needs and collaborate with stakeholders to improve care plans and outcomes
Required Qualifications and Education
- 4+ years of experience in care planning or case management
- Experience working with the elderly or adults with disabilities
- Remote case management experience is highly desired
- Comprehensive knowledge of industry concepts and principles
- Multilingual skills, particularly in Spanish, are an asset
