Key Responsibilities

• Engage and educate caregivers via phone and digital technology to enhance their wellbeing
• Manage a caseload and utilize technology for effective coaching and resource referral
• Assess caregiver needs and collaborate with stakeholders to improve care plans and outcomes

Required Qualifications and Education

• 4+ years of experience in care planning or case management
• Experience working with the elderly or adults with disabilities
• Remote case management experience is highly desired
• Comprehensive knowledge of industry concepts and principles
• Multilingual skills, particularly in Spanish, are an asset
