Key Responsibilities

• Engage and enroll patients through telephonic outreach, serving as their primary point of contact
• Develop and implement care plans in collaboration with the interdisciplinary care team, ensuring comprehensive support
• Advocate for patients by identifying barriers to care and connecting them with necessary resources

Required Qualifications

• 3 years of experience in healthcare coordination or community health work
• Experience with a team-based approach to care
• Strong knowledge of community resources
• Ability to work effectively in a remote environment with required internet speed
• Intermediate skills with MS Office Suite, including Outlook and Teams
