Job Summary
A company is looking for a Care Transitions Clinical Administrative Coordinator.
Key Responsibilities
- Manage member referral creation from admissions and discharges to ensure continuity of care
- Document pertinent information regarding admissions and discharges from inpatient settings
- Follow policies and procedures aligned with regulatory and organizational standards
Required Qualifications
- High School Diploma / GED OR equivalent work experience
- 1+ year of healthcare experience and/or social work/community outreach/advocacy experience
- 1+ year work with the Medicare and/or Medicaid, DSNP, CSNP Population
- Experience using Microsoft Word, Excel, and Outlook
- Ability to work full time Monday - Friday with flexibility for 8-hour shifts
