A company is looking for a Community Health Worker to support patient care and connect members with community resources in a remote setting.

Key Responsibilities

• Create positive relationships with members and engage them in managing their health
• Coordinate care plans and connect members to social services and community resources
• Advocate for members and ensure compliance with health-related regulations and quality metrics

Required Qualifications

• High School Diploma/GED or higher
• 3+ years of experience in a community health setting
• Intermediate computer proficiency, including Microsoft Office applications
• Residence in Texas with access to a designated quiet workspace and high-speed internet
• Ability to secure Protected Health Information (PHI)
