Compact Licensed Transition of Care Nurse

Job Summary

A company is looking for a Transition of Care Nurse.

Key Responsibilities

• Outreach to caregivers and patients after hospital discharge for a Transition of Care visit
• Review discharge plans with caregivers and assess barriers post-discharge
• Coordinate care between patients and their PCP/specialists to ensure continuity of care

Required Qualifications

• Bachelor's in Nursing from an accredited university
• Compact Licensed RN or eligibility for other state licenses
• Minimum of 2 years direct clinical experience in an outpatient setting, Emergency Department/Triage, Complex Care, or PICU preferred
• Experience with pediatric population required
• Bilingual in English and Spanish required
