Job Summary

A company is looking for a Health Coach to join their team.

Key Responsibilities

• Motivate, focus, and energize participants using a mobile care management platform
• Establish goals and create achievable care plans with participants
• Develop collaborative relationships with participants and their care teams to improve health outcomes

Required Qualifications

• Bachelor's Degree in a relevant field (e.g., Community Health, Public Health, Nursing)
• Health coaching certification with accreditation (e.g., CHES, ACE, ICF)
• Minimum 2-3 years of health coaching experience
• 1-3 years of experience in chronic disease management
• Must possess valid Business License and current insurance policies
