Health Coach

Job Summary

A company is looking for a Health Coach to empower patients with complex medical needs to actively manage their health.

Key Responsibilities

• Serve as a patient health coach on an interdisciplinary care team
• Engage with patients to set health and wellness goals and provide support for sustainable changes
• Document services provided and collaborate with healthcare providers as needed

Required Qualifications

• Bachelor's degree in healthcare or related field preferred
• Minimum of 3 years' experience in chronic condition clinical coaching or relevant healthcare coaching experience
• National Board for Health and Wellness Coaching (NBHWC) Certified preferred
• Comfortable with digital technology and various EMRs
• Ability to build strong relationships with patients and work within an interdisciplinary team
