Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Behavioral Health Case Manager to support individuals with behavioral, emotional, and mental health needs through comprehensive care coordination and case management.

Key Responsibilities

• Manage a caseload of members with behavioral health conditions
• Provide care coordination and support during transitions of care

Required Qualifications

• Minimum 1 year of experience in behavioral health, social work, or a community-based role
• Strong computer skills and comfort navigating multiple systems
• High school diploma or GED

Preferred Qualifications

• Active license as a Social Worker (LMSW, LCSW, or equivalent)
