Job Summary

A company is looking for a Clinical Care Manager RN.

Key Responsibilities

• Provides outreach and engages with enrollees referred for the Care Partner program
• Conducts comprehensive assessments of enrollees' medical, psychiatric, and social issues
• Coordinates the development and implementation of enrollee care plans and monitors their health status

Required Qualifications

• Bachelor's degree in nursing required; master's in nursing preferred
• Registered Nurse (RN) license required
• Minimum of 5 years clinical and case management experience preferred
• Experience working with individuals with psychiatric disabilities and co-occurring disorders preferred
• Preference for bilingual/bi-cultural applicants and those with lived experience of psychiatric conditions
