Job Summary

A company is looking for a Social Worker - Memory Care.

Key Responsibilities:

• Provides assessments and counseling, referring patients to necessary resources
• Evaluates patient and family support systems to access community resources
• Coordinates care plans with healthcare professionals to ensure continuity of care

Required Qualifications:

• Masters of Social Work degree (required)
• Active Massachusetts' LCSW or LICSW licensure (required)
• Minimum one year of experience as a practicing social worker in a healthcare or social service setting
• Experience in Memory Care or Geriatric Care (preferred)
• Bilingual language skills are optional
