Nebraska Licensed Care Manager (RN)

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers and ensure optimal outcomes
• Coordinate with providers and community resources to implement and monitor personalized care plans
• Facilitate education for members and families on healthcare options and provide resource support

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in nursing
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• 3+ years of clinical nursing experience in acute care settings strongly preferred
• 2+ years of case management experience in a managed care environment is a plus
