Job Summary

A company is looking for a Proactive Titration Licensed Practical Nurse.

Key Responsibilities

• Contact patients to discuss medication titration recommendations and ensure adherence to treatment plans
• Maintain accurate documentation of patient interactions in compliance with HIPAA and clinical protocols
• Provide empathetic communication with patients and healthcare providers while managing responsibilities independently

Required Qualifications

• Licensed Practical/Vocational Nurse (LPN/LVN) with compact licensure
• 5+ years of clinical experience in acute or ambulatory care, preferably in cardiology or nephrology
• Experience maintaining HIPAA-compliant environments
• Must have a HIPAA-compliant home office setup with reliable internet
• Strong track record of working independently and managing time effectively
