New York Licensed Care Management Director

Job Summary

A company is looking for a Director of Care Management to oversee care management operations and strategic initiatives.

Key Responsibilities

• Lead the strategic development of analytics and business intelligence for population health initiatives
• Manage daily operations of the Care Management team, including performance metrics and staff training
• Collaborate with internal and external partners to improve communication and care management services

Required Qualifications, Training, and Education

• Bachelor's degree, preferably in Nursing
• Current unrestricted New York State RN license required
• CCM certification required, with consideration for current enrollees
• 10+ years of relevant work experience, preferably in a healthcare provider setting
• 5 years of clinical experience with health plan experience highly desirable
