New York Licensed Senior Care Manager

Job Summary

A company is looking for a Senior Care Manager (RN) to assess, plan, and implement care management activities for members.

Key Responsibilities

• Develop and continuously assess personalized care plans in collaboration with healthcare providers
• Coordinate care between members, families, and care provider teams to ensure appropriate services
• Monitor member status and revise care plans as necessary to meet goals and address unmet needs

Required Qualifications

• Active and unencumbered NY RN Licensure required
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience in care management
• Experience in collaborating with healthcare providers and managing care plans
• Ability to provide education to members and their families on healthcare options
