Key Responsibilities

• Conduct comprehensive health assessments and develop care plans for members
• Evaluate risk scores and assess gaps in care while collaborating with external providers
• Provide clinical insights and support to the care team, ensuring high-quality documentation practices

Required Qualifications, Training, and Education

• At least 3 years of Nurse Practitioner experience, including 2 years in adult oncology
• Minimum 1 year of experience in telehealth
• Possession of a compact RN license and an unrestricted Advanced Practice license
• Ability to adapt to a fast-paced, startup environment
• Experience with technology, including video chatting and electronic health records
