New York State Licensed Care Manager

Job Summary

A company is looking for a Care Manager, Bilingual Spanish/English - Remote.

Key Responsibilities:

• Advocates for and educates beneficiaries on services and self-management techniques
• Conducts assessments to identify barriers and develops care plans aligned with treatment goals
• Coordinates with multi-disciplinary health care teams to ensure person-centered care and maximize health outcomes

Required Qualifications:

• NYS RN or LCSW/LMSW (any state)
• Bilingual in English and Spanish
• Experience managing member information in a shared network environment
• Relevant work experience preferably as a Care Manager
• Knowledge of community health practices for frail adults and cognitive impaired seniors
