Job Summary

A company is looking for an RN Care Manager - Pop Health.

Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate care transitions across various healthcare settings and facilitate multidisciplinary team meetings
• Collaborate with payers and ensure accurate documentation and billing for services rendered

Required Qualifications

• Graduation from a state accredited school of professional nursing
• Active RN license to practice in North Carolina
• Minimum of 2 years of experience as a registered nurse
• BSN required or must be enrolled in an accredited program within 4 years of employment
• Strong knowledge of risk segmentation and patient-centered care planning
