Job Summary

A company is looking for a Triage Registered Nurse - Home Health Remote PT Weekend Variable.

Key Responsibilities

• Collect and analyze healthcare data from telephonic contacts and EMR information
• Develop and implement care plans to promote patient health and safety
• Coordinate care delivery and evaluate progress towards patient outcomes

Required Qualifications, Training, and Education

• Graduate from an accredited School of Nursing; BSN preferred
• Current RN license or temporary license in the state of practice
• Basic Life Support for Healthcare Provider certification
• Additional education, training, or certifications may be required
• Physical ability to perform nursing tasks and respond to patient conditions
