Nurse Practitioner Telehealth (Multi-State)

Job Summary

A company is looking for a Nurse Practitioner - PRN Telehealth.

Key Responsibilities

• Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, and documentation
• Collaborate with physicians, case managers, and other healthcare professionals for patient care
• Provide services through home visits, skilled facility visits, and telehealth visits

Required Qualifications

• Master's Degree or equivalent experience and NP licensure
• Current RN and NP licensure with prescription authority
• Board certification in a Matrix approved specialty
• Current BLS, ACLS, or CPR certification
• Minimum of 1 year experience as a Nurse Practitioner
