Key Responsibilities
- Proactively reach out to patients who have missed appointments to reschedule and re-engage them
- Maintain accurate chart hygiene and prepare documentation for follow-up appointments
- Collaborate with clinical teams to support ongoing patient care and address care gaps
Required Qualifications
- Passion for working with seniors, their families, and caregivers
- Bilingual in Spanish
- 1+ years of experience in a customer service, administrative, or healthcare support role
- Familiarity with HIPAA or prior experience in a healthcare setting (preferred)
- Comfort navigating basic technology (e.g., phone systems, EMRs, scheduling tools)
