Job Summary

A company is looking for a Bilingual Remote Care Coordinator (LPN).

Key Responsibilities
  • Conduct monthly CCM phone calls with patients and healthcare providers to ensure patient needs are met
  • Create and revise personalized care plans in coordination with healthcare practices
  • Identify social determinants of health and collaborate with community resources to address patient needs
Required Qualifications
  • At least 1 year of experience as an LPN with an active professional license
  • Exceptional verbal and written skills in English and Spanish
  • Experience with multiple EMR/EHR systems and familiarity with Microsoft Office and Google Suite
  • Highly self-motivated and organized, capable of working independently
  • Experience serving a geriatric population and in case management or care coordination is a plus
