Job Summary

A company is looking for a Care Manager (RN) (State).

Key Responsibilities

• Evaluate member needs and develop personalized care plans to address barriers to care
• Coordinate care between members, families, and healthcare providers to ensure effective service delivery
• Monitor member progress and update care plans as necessary to meet changing needs

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Must reside in California
