Job Summary

A company is looking for a Care Continuum Partner who serves as the main liaison between patients, families, and the clinical team throughout the continuum of care.

Key Responsibilities

• Collaborates with the clinical care team to ensure smooth operation and patient flow through pre-visit planning
• Coordinates and schedules complex appointments across various services, ensuring comprehensive care for patients
• Assesses patient and family needs, supports care plan compliance, and facilitates communication among the care team and patients

Required Qualifications

• High School Diploma/GED with a commitment to advancing education
• 2 years of experience interacting with patients in a healthcare setting
• 3 years of experience in a complex health system environment or related field
• Working knowledge of basic computer functionality
• Ability to handle numerous tasks simultaneously and maintain strict confidentiality
