Key Responsibilities
- Provide care navigation support for patients, connecting them with providers and social services
- Monitor the member support phone line and address patient inquiries
- Manage patient scheduling, eligibility checks, and referrals to the Case Management team
Required Qualifications
- 4+ years of healthcare administration experience, preferably in virtual care
- Familiarity with social determinants of health (SDOH) platforms and referrals
- Results-oriented with a fast-paced operational approach
- Ability to work cross-functionally with clinical teams
- Passionate about maternal and child health
