- Coordinate the delivery of services and maintain accurate documentation in the client portal
Required Qualifications
- Bachelor's degree in Social Work, Human Services, Healthcare Administration, or related field
- 3-5 years of direct service experience in a person-centric environment
- Experience in remote care coordination, digital navigation, and case management
- Commitment to serving diverse populations with empathy and respect for cultural differences
- Understanding of health support systems and trauma-informed practices
