Certified Medical Assistant Coordinator

Job Summary

A company is looking for a Bilingual Remote Care Coordinator (Certified Medical Assistant).

Key Responsibilities

• Conduct monthly phone calls with patients and healthcare providers to ensure patient needs are met
• Create and revise personalized care plans in coordination with healthcare practices
• Identify social determinants of health and collaborate with community resources to address patient needs

Required Qualifications

• At least 1 year of experience as a Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) with active certification
• Exceptional proficiency in English and Spanish
• Experience with EMR/EHR systems and familiarity with Microsoft Office and Google Suite
• Highly self-motivated and organized, capable of working independently
• Experience with geriatric populations and care coordination is a plus
