Key Responsibilities

• Utilize a collaborative process for assessment, planning, facilitation, and advocacy for health needs
• Navigate multiple computer systems to manage case information effectively
• Adapt to a fast-paced environment while multitasking and prioritizing tasks

Required Qualifications

• RN with unrestricted active license in Florida
• 2-3 years of clinical practice experience in a hospital or alternative care setting
• 2 years of experience in the healthcare and/or managed care industry
• Case Management experience required
• High School Diploma or equivalent; Case Management Certification preferred
