Job Summary

A company is looking for an Electronic Referral Specialist - Remote.

Key Responsibilities

• Serve as the first line of customer service for external and internal customers
• Receive and document patient referrals to home health services through electronic referral systems
• Communicate pertinent patient information to the care center team and follow up with Business Development regarding pending admissions

Required Qualifications

• High School diploma or GED equivalent
• Experience in a timely and quality-focused work environment
• Home health or hospice experience preferred
• GA nursing license or compact nursing license (LPN/RN) is highly preferred
• EMR experience is required
