Job Summary

A company is looking for an LCSW Behavioral Health Case Manager.

Key Responsibilities

• Engage members to identify behavioral health risk factors and provide education on self-management and treatment adherence
• Plan, implement, coordinate, and monitor case management activities, facilitating access to care and removing barriers
• Collaborate with members, physicians, and care teams to support health care goals and promote healthy lifestyles

Required Qualifications

• 4+ years of related work experience in Managed Care, Home Healthcare, or Community Healthcare
• Master's degree or equivalent work experience in Social Work, Human Services, Psychology, or a related field
• LCSW - State Licensed Medical Social Worker
• Experience in the Healthcare Industry and Managed Care preferred
• Experience working with adult/geriatric populations
