Licensed Behavioral Health Case Manager

Job Summary

A company is looking for a Case Manager - Behavioral Health - Medicaid.

Key Responsibilities

• Conducts comprehensive clinical assessments and collaborates with members to develop care plans
• Identifies and facilitates referrals to relevant resources and monitors care plans over time
• Engages with providers to ensure alignment of treatment plans with benefits and care coordination

Required Qualifications

• Registered Nurse with psychiatric experience or a licensed Behavioral Health Professional
• 3 years of relevant direct clinical care experience
• CCM Certification or ability to obtain within 3 years
• High school diploma (or equivalency) and legal authorization to work in the U.S
• Flexibility to work varied hours
