Job Summary

A company is looking for a Medical Director - Post-Acute Care Management - Care Transitions.

Key Responsibilities

• Provide daily utilization oversight and communication with network physicians and hospitals
• Conduct peer-to-peer clinical case reviews and manage provider discussions to enhance healthcare delivery
• Collaborate with leadership and client services to develop strategic initiatives and improve care coordination

Required Qualifications

• Board certification as an MD, DO, or MBBS with a current unrestricted medical license
• Ability to obtain licensure in multiple states
• 3+ years of post-residency patient care experience, preferably in inpatient or post-acute settings
