Job Summary

A company is looking for a Nurse Care Manager.

Key Responsibilities

• Manage patients' specialized needs in collaboration with physicians and care teams
• Develop and implement comprehensive care management plans based on patient assessments
• Coordinate patient care delivery and monitor the effectiveness of case management plans

Required Qualifications

• Current Registered Nurse license is required; Bachelor's of Science in Nursing preferred
• Certified Case Manager (CCM) credential preferred
• 3+ years of clinical practice in a healthcare setting highly preferred
• Prior experience with electronic health records or health registries required
• Microsoft Office skills, including Excel, Word, PowerPoint, and Outlook required
