New York RN Case Manager

Job Summary

A company is looking for an RN Case Manager.

Key Responsibilities

• Conduct initial intake calls and ongoing care coordination for complex patients
• Develop and implement care plans in collaboration with the patient's care team
• Support patients during care transitions and ensure alignment with their needs

Required Qualifications

• Current RN License in good standing in the state of practice
• Bachelor's degree in nursing or associates in nursing with a related bachelor's degree
• Minimum of 4 years clinical experience with at least 2 years in case management
• Certified Case Manager (CCM) certification or commitment to obtain
• Geriatric care experience is highly desired
