Non-Clinical Patient Navigator

Job Summary

A company is looking for a Patient Navigator (Non-Clinical).

Key Responsibilities

• Develop relationships with patients through telephonic outreach following clinical events or diagnoses
• Collaborate with the clinical team and utilize health system resources to enhance patient experiences
• Research and identify community resources to assist patients and contribute to the navigation program's development

Required Qualifications, Training, and Education

• Bachelor's degree preferred, or equivalent professional experience considered
• Proficiency with personal computers and experience with Microsoft Excel preferred
• Technical and analytical skills required
• Ability to work in a private HIPAA compliant workspace
• Must have broadband internet access with specified download and upload speeds
