Job Summary

A company is looking for a Community Health Worker - Perinatal Outreach.

Key Responsibilities

• Coordinate and follow-up on referrals to community organizations for clients
• Provide health education and teach self-advocacy skills to clients
• Document client interactions and report on productivity and service barriers

Required Qualifications

• High School Diploma or GED is required
• Ohio CHW certificate preferred; certification required within 12 months of hire
• Minimum of 3 years of relevant experience
• Valid Ohio driver's license with one year of driving experience and no traffic citations
• Sensitivity and experience working within diverse cultures
