Job Summary

A company is looking for a Bilingual Care Coordinator.

Key Responsibilities

• Conduct outreach to educate members about program benefits and assess their needs
• Develop care plans with members and manage a caseload to ensure timely contact and care coordination
• Document actions taken in a care management platform and facilitate communication with healthcare professionals

Required Qualifications

• Fluency in English and Spanish
• 2+ years of experience in care coordination, case management, or related fields
• Experience working with Medicaid and/or Dual Eligible patients
• Remote work experience preferred
• Dedicated home office for remote work
