Job Summary
A company is looking for a Transition of Care Support Coordinator.
Key Responsibilities
- Receives and responds to Transition of Care referrals and manages work queues
- Completes stratification tools and conducts outbound calls to members and providers
- Maintains customer service standards and assists with workflow process improvements
Required Qualifications
- 3+ years of related experience in a relevant field
- High school diploma or equivalent and legal authorization to work in the U.S
- Proficient in MS Office Suite, including Access
- Experience in customer service or call center environments preferred
- Knowledge of Medicare and Medicaid is a plus
