South Carolina Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical health needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers and facilitate optimal healthcare outcomes
• Coordinate with providers and community resources to ensure members receive appropriate care and services
• Monitor member progress and revise care plans as necessary, providing education and support to members and families

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in nursing or care management
• RN - Registered Nurse State Licensure and/or Compact State Licensure required
• Must reside in the state of South Carolina
