Job Summary

A company is looking for a Manager of Post-Acute Care Operations - Population Health.

Key Responsibilities

• Develops and oversees the professional practice model for post-acute care management
• Coordinates care across various post-acute sites and acts as a point of contact for stakeholders
• Participates in quality improvement initiatives and integrates programming into care delivery

Required Qualifications, Training, and Education

• Current RN License in the state of practice or covered by compact (required)
• Bachelor of Science in Nursing (BSN) (required)
• 3 years of experience in transitions of care or care management (preferred)
• Care Management certification (preferred)
• Master's degree in Nursing or Healthcare Administration (preferred)
