UM Care Specialist

Job Summary

A company is looking for a UM Care Specialist - REMOTE.

Key Responsibilities

• Execute non-clinical aspects of the Care Management process and ensure accurate data entry
• Provide telephonic outreach to members for care management support and coordinate with the interdisciplinary care team
• Manage caseloads and assist with care interventions under the direction of a Care Manager

Required Qualifications, Training, and Education

• Bachelor's Degree
• Medical Assistant certification preferred
• 1 - 2+ years of client-facing or data entry experience in a health care environment
• Experience in a highly electronic environment
• Willingness to work a flexible schedule including weekends, nights, and holidays as needed
