Job Summary

Key Responsibilities

• Understand and address the needs of stroke survivors transitioning from hospital to home
• Request and organize medical records and coordinate with healthcare providers for patient care
• Provide technical and customer support for stroke survivors and care partners, including managing telehealth visits

Required Qualifications

• 3+ years of experience in a clinic or hospital-based role
• Skilled in patient support and navigating healthcare systems
• Proficient in using electronic medical records (EMRs) and telehealth systems
• Experience providing technical support for digital tools used in patient engagement
• Commitment to cultural competence and inclusivity in patient care
