A company is looking for a Care Coordinator (contract) Bilingual (English & Spanish).
Key Responsibilities
- Conduct needs assessments and screeners across various parameters
- Manage care plans and referrals, acting as a liaison between members and care management teams
- Promote member engagement and advocate for their physical, behavioral, and social care needs
Required Qualifications
- Bilingual in English and Spanish
- 3-5 years of experience in social work, nursing, or a healthcare-related field
- Knowledge of referral coordination to community and private/public resources
- Experience in relationship management
- Highly organized and comfortable managing multiple workstreams concurrently
