Bilingual Care Coordinator

Job Summary

A company is looking for a Care Coordinator (contract) Bilingual (English & Spanish).

Key Responsibilities

• Conduct needs assessments and screeners across various parameters
• Manage care plans and referrals, acting as a liaison between members and care management teams
• Promote member engagement and advocate for their physical, behavioral, and social care needs

Required Qualifications

• Bilingual in English and Spanish
• 3-5 years of experience in social work, nursing, or a healthcare-related field
• Knowledge of referral coordination to community and private/public resources
• Experience in relationship management
• Highly organized and comfortable managing multiple workstreams concurrently
