Job Summary

A company is looking for a Part Time Case Management, RN.

Key Responsibilities

• Conduct comprehensive assessments of patients' physical, psychosocial, emotional, spiritual, environmental, and financial needs
• Develop strategic treatment plans in collaboration with patients, caregivers, and multidisciplinary healthcare providers
• Monitor and evaluate patient outcomes, ensuring effective intervention and quality patient care

Required Qualifications

• Graduation from an accredited Registered Nursing (RN) program
• Current California RN license; multi-state license also required
• Minimum 5 years of medical/surgical or acute care experience, including 2 years in case management
• Knowledge of medical claims and relevant coding (ICD-10, CPT, HCPCS)
• Ability to work independently and manage multiple cases effectively
