Job Summary
A company is looking for a Cardiac Rehab Patient Retention Specialist.
Key Responsibilities:
- Serve as the primary contact for at-risk patients, building rapport and encouraging participation
- Implement and optimize retention strategies while tracking patient progress and engagement
- Address patient concerns and escalate urgent issues as necessary
Qualifications:
- Bachelor's or Master's degree in Health Education, Communications (with healthcare experience), or Exercise Science/Physiology
- Preferred: Bachelor's degree in a health-related field or certification as a Health Coach or CHES
- 1-3+ years of experience in patient engagement, case management, or healthcare customer service
- Experience in cardiac rehab, chronic disease management, or a virtual setting is preferred
- Experience with motivational interviewing or health coaching is a strong plus
