Cardiac Rehab Patient Retention Specialist

Job Summary

A company is looking for a Cardiac Rehab Patient Retention Specialist.

Key Responsibilities:

• Serve as the primary contact for at-risk patients, building rapport and encouraging participation
• Implement and optimize retention strategies while tracking patient progress and engagement
• Address patient concerns and escalate urgent issues as necessary

Qualifications:

• Bachelor's or Master's degree in Health Education, Communications (with healthcare experience), or Exercise Science/Physiology
• Preferred: Bachelor's degree in a health-related field or certification as a Health Coach or CHES
• 1-3+ years of experience in patient engagement, case management, or healthcare customer service
• Experience in cardiac rehab, chronic disease management, or a virtual setting is preferred
• Experience with motivational interviewing or health coaching is a strong plus
