Job Summary

A company is looking for a Care Advisor, Care Concierge.

Key Responsibilities

• Assist members with assessments and recommend service plans based on their needs
• Coordinate and schedule services while providing follow-up support to members
• Help clients access community resources such as homecare, meals, and transportation

Required Qualifications

• High School Diploma preferred
• Experience in Public/Community Health or related fields preferred
• Experience in social services, care coordination, or healthcare navigation is a plus
• Must be 18 years of age or older
• Computer proficiency and knowledge of clerical functions required
